Two things have become increasingly apparent during recent times Our health needs to be priority #1 and our community needs our help. Congrats on helping with BOTH! The first link is our intake paperwork (adult and pediatric). It should only take 5-10 minutes to fill out and can be done entirely from your phone, tablet, or computer!
